The 1099-G form in Illinois reports your gross unemployment compensation received for the tax year. Forms are mailed out to claimants by January 31 following the end of the tax year. If you did not receive form 1099-G or you need a copy, and the Illinois Department of Employment Security has a current address listed for you, you can re-order a 1099-G for specific years and receive the form within one to two business days.

Call 888-337-7234 between the hours of 5 a.m. and 7:30 p.m. Central Standard Time Monday through Friday to use the automatic service.

Select your preferred language by pushing “1” on the phone keypad for English or “2” for Spanish.

Listen to the automatic operator, but press “3” on the phone keypad to check status of your claim.

Press the numbers on the keypad to enter your Social Security number. Enter your PIN using the keypad. Your PIN is the number you use to file your weekly claims. The PIN never changes unless you change it yourself from the very first time you registered with IDES for unemployment benefits.

Push number “3” on the keypad to indicate you want a report of benefits reported to the IRS after the automatic operator accepts your PIN. The automatic system will ask you what year 1099-G form you are requesting. Enter the year by using the keypad on the phone. For instance, if you need 1099-G form for 2008, enter 2-0-0-8 on the keypad and the department will send you the 1099-G form from 2008 in one to two business days.
